In November 2005, NHS Networks was asked by the Department of Health to request examples of good practice from all the Primary Care Trusts (PCTs) in England. The Your Health, Your Care, Your Say consultation had highlighted that the public wanted primary care to: focus more on prevention; be better integrated with social care and other services; offer services which have been traditionally provided in hospital settings; and provide rapid access. PCTs were asked to submit examples of projects which reflected one or more of these elements and more than 100 case studies were received. http://www.networks.nhs.uk/ideas/index.php
