This community health centre in Winnipeg undertakes performance appraisals every two years. It developed its own tool that addresses soft skills such as teamwork, interpersonal conflict, etc. The performance appraisal process involves team members listing their roles/responsibilities with the six top tasks that want to be evaluated on. The process also includes peer review, self evaluation and includes salaried physicians.

This is one of the oldest and most wellestablished interdisciplinary medical practices in Ontario, having existed since the mid-1970s in the east end of Hamilton. Its performance appraisal and development plan includes objectives, performance summary in meeting those objectives, general skills appraisal section, and identification of the employees training and development needs.
