Chinook Primary Care Network Communications Plan
the Chinook Health Region in southwestern Alberta has prepared a comprehensive
communications plan to get its messages about primary health care out to
multiple stakeholder groups.

Dr. Charles L. LeGrow Health Care Centre (site visit)
This multi-functional
district health centre is located in the town of Port aux Basques on Newfoundland's
southwest coast. This centre participates in a Primary Health Care Enhancement
(PHCE) project and has a large interdisciplinary and intersectoral working group,
the Southwest coast interdisciplinary working group (SID) terms of reference. The
terms of reference of both are included. It also held a weekend retreat with primary
health care professionals to facilitate communication and solve conflict related to
scope of practices. A documentation of the process followed is also included.

This primary care group is part of the Chinook Health Region that is located in the mixed rural and
urban area of southwestern Alberta, adjacent to British Columbia and the northern
boundary of the United States. This centre has adopted a "Consent to Counseling"
form, which spells out the roles and responsibilities of patients, psychologist, and
physicians involved in the care, especially regarding transfer of information and
confidentiality
